Just a follow-up note--I contacted Woodward Resource Center's Medical Records Department. They sent me a release form, which I completed and faxed back to them. About a week later, I received a packet in the mail containing copies of the records of my relative's stay at Woodward State Hospital from her admission on September 29, 1917 until her death on November 26, 1924. The records included her original admission application and health history, ward records of observations by attending staff, and records of her death at the facility. This is, frankly, far more information than I ever thought I'd find. And while the story told by the records is very sad, I'm grateful to have them in hand.
