SIOUX CITY | David Michael Bolfik, 72, of West Des Moines, formerly of Sioux City, passed away on Feb. 16, 2017, after a courageous battle with Lewy Body Dementia.

A celebration of life mass will be noon Friday, Feb. 24, at Sacred Heart Catholic Church in West Des Moines. Burial will be in Resthaven Cemetery. Arrangements are under the direction of McLaren's Funeral Chapel in West Des Moines.

Dave is survived by his wife, Susan; daughter, Debi Bolfik (Dustin Ennis); son, Michael (Jennifer) Bolfik; two grandchildren, Gage and Caydence Bolfik; brothers, Larry Bolfik (Erma Dean) of Tempe, Ariz., and Tom (Karen) Bolfik of Savannah, Ga.; sisters, Mary Lou Bolfik of Pikesville, Mo., and Celeste (Steven) Froehlich of Montclair, Va.; sister and brothers in-law, Sandy (John) Graser of Sioux City, Denny (Judy) Ray of Mesa, Ariz., and Patti Nessessen of Mesa; his best man, Jim (Connie) Galles of Sioux City; cousins, Barb (Bill) Berens; and many nieces and nephews.

In lieu of flowers, memorial contributions can be directed to the family, HCI Kavanaugh House or Lewy Body Dementia Association.
