Phyllis Jean Shoars Dvorak, 78, of Overland Park, Kansas, and formerly of Waterloo, Iowa, was born on March 18, 1933 in Osage, Iowa. She was the daughter of Smith Lester and Grace Edith Lewis Shoars. Phyllis married Donald C. "Cookie" Dvorak in Cresco, Iowa, on February 25, 1950. Cookie preceded Phyllis in death on August 1, 1999.

Phyllis enjoyed cross-stitching, playing cards, dancing, socializing and wintering in Texas. She devoted her life to her family as a homemaker.

Phyllis died on Wednesday, December 28, 2011 at the Delmar Gardens Nursing and Rehabilitation Center under the care of Kansas City Hospice of natural causes.

She was preceded in death by her husband, Donald.

Phyllis is survived by her children, Douglas (Cheryl) Dvorak of Overland Park, KS, Terrance (Susie) Dvorak of Norco, Calif., LaDonna (David) Noelting of Lansing, Iowa, and Gregory (Bonnie) Dvorak of Schertz, Texas, 12 grandchildren and 12 great-grandchildren.
