Johan (John) Theodore Maakestad was born July 7, 1894 at Cedar, Mitchell, Iowa to Syver Johan Maakestad and Antonette Docken, the third of their six children.

He was baptized on August 12, 1894 at the Rock Creek Lutheran Church at Meroa, Cedar, Mitchell, Iowa. His baptismal sponsors were: Mr & Mrs Anton Iverson; Oline Maakestad and Gabriel Docken. He was confirmed at RCLC on June 6, 1909. He served in the Armed Forces.

Johan married Mabel Koch at Iowa.
