Hazel Evans, 84, a former Winterset resident died of heart failure March 30, 2001, at the Syverson Lutheran Home in Eau Claire, Wis.
A memorial service will be held at 1 p.m. Sunday, April 29 at Grace United Methodist Church, 3700 Cottage Grove, Des Moines.
Hazel Evans was born Aug. 30, 1916 to Martin and Inez Uggen of Epping, N.D. She graduated from Wells High School in Wells, Minn., in 1933 and from Winona State Teachers College in Winona, Minn., in 1937. Hazel taught in the Lake City, Minn., and Sioux City school districts. In 1946, she married Dr. John E. Evans. They moved to Winterset in 1953 where they lived and raised their family before moving to Des Moines in 1976. In 1999, Hazel moved to Eau Claire. She was a member of Grace United Methodist Church, PEO Chapter JY, and the Des Moines Women's Club.
She was preceded in death by her husband and two brothers.
She is survived by a son, Dr. John M. Evans (wife JoAnne) of Eau Claire; a daughter, Jane Crise (husband Bill) of Milwaukee, Wis.; a sister Mildred Larson of Sioux City and five grandchildren.
