Death Certificate

Aloha, I have a blurry "Standard Certificate of Death" for Linn County, Iowa circa 1924, with 20 items that are filled out. I can read the information OK but no all 20 of the 'headings' you can fill out. Is there a blank form (like census forms) that I can get a copy of to better be able to read the 20 headings on this form? Thank you.
