I'm not aware of any "Fort Fairfield" connection to Fairfield, Iowa. There were soldiers who served in the Civil War whose roots came from Fairfield, but nothing I've read suggests that there was a garrison or "Fort" that would be associated with the town/city of Fairfield. Without seeing the death record itself, one would have to wonder if the listing was just a clerical error or misunderstanding when it was written up.
