Phyllis McINTOSH

Phyllis McINTOSH, 79, of Windsor Heights and formerly of Fairfield, died Thursday, May 28, 2015, at the Iowa Jewish Senior Life Center in Des Moines (note: in Polk Co., IA).

The funeral will be at 2:30 p.m. Tuesday at Merle Hay Funeral Home in Des Moines.

Visitation will begin at 1 p.m. Tuesday at the funeral home.

Survivors include: her husband, Floyd; two children: Laura BOTTS and husband Bill of Ankeny and Brian McINTOSH of Cleveland, Tennessee; three grandchildren; five great-grandchildren; and five siblings: Donald, Patricia, Lillian, Philip and Billy.

She was preceded in death by: her parents, Leo and Hazel GARRETT; and two brothers Maynard "Bo" and Leo Jr.
