John Willie "J.W." Perkins, 57, of Prairie Ridge Care and Rehabilitation in Mediapolis, formerly of Burlington died at 9:30 p.m. Sunday, May 4, 2003 at Great River Medical Center in West Burlington.

Born July 25, 1945 in Lake Village, Arkansas, he was the son of Tom And Willie Bea Collins Perkins.

Mr. Perkins worked as a forklift operator for Raider PreCast. He was of the Baptist faith. He enjoyed playing cards, dominos, fishing and loved his cars.

Survivors include one son, William Perkins of Iowa City; one daughter, Nora Perkins of Gary, Ind.; two granddaughters; his stepmother, Ella Bea Tate of Des Moines; four brothers, Willie Perkins of Pine Bluff, Arkansas, Tom Perkins and Johnny Ray perkins of Crossett, Arkansas; and Ed Perkins of Burlington; three stepbrothers, Robert Milsap,James Milsap and Gregory Taylor all of Des Moines; one sister, Annie B. Scott of Crossett, Arkansas; two stepsisters, Beatris Taylor and Diane Mitchell, both of Des Moines; and numerous nieces, nephews and cousins.

He was preceded in death by his parents and one brother.

Visitation will be from 6 to 7:30 p.m. Thursday in Prugh's Chapel.

The funeral for Mr. Perkins will be a 1 p.m. Friday at Prugh's Chapel with Elder Fred Starling officiatiing. Burial will be in Aspen Grove Cemetery.
