Terry Van Slyke, 41, Arvada, Colo., and formerly of Boone, died Tuesday at Arvada, Colo.

Funeral services will be held at 10 a.m. Monday at Stark-Welin Chapel in Boone with Diaconal Minister Diana Sickles officiating. Burial will be in Linwood Park Cemetery in Boone.

Friends may call at Stark-Welin Chapel in Boone Sunday from 7 to 9 p.m. The family will be present to receive friends from 7 to 8:30 p.m. Visitation continues Monday at the chapel from 8 a.m. until time of services.

She was born Jan. 13, 1962, in Lake Forest, Ill., the daughter of James A. and Elsie (Kozak) Faulkner. She attended Boone schools and had lived and worked in the Denver, Colo. Area for the past 20 years. Her hobbies included gardening and crafts.

Survivors include her mother and step-father, Elsie and Darwin Schuett, of Boone; one sister and brother-in-law, Barbara and Ed Picard of Arvada, Colo.; two step-brothers, Robert Schuett and his wife, Doreen, of Charlottesville, Va., and Richard Schuett and his wife, Denise, of Ankeny; three step-sisters, Dona Koelling and her husband, Gary, of Mundelein, Ill., Dana Deacon and her husband, Graham, of Greensboro, N.C., and Lisa Moe and her husband, Jodi, of Charlotte, N.C.; one half-sister, Dawn Gilliam and her husband, Ken, of Carol Stream, Ill.; and numerous aunts, uncles, nieces and nephews.

She was preceded in death by her father; her step-mother, Ruth Faulkner; and her grandparents.

Memorials may be given to the Boone Area Humane Society.

Stark-Welin Funeral Directors of Boone is in charge of arrangements.
