CENTERVILLE — Phillip T Lewis, 58, died at 1:05 a.m. Sunday at his home, 636 North Main, following two years failing health. He had been bedfast since undergoing a major operation July 3.

He was born at Mendota, Mo., March 16, 1888, the son of John D. and Alice Lewis, He was mar¬ried at Rathbun to Miss Minerva Blanche Mincks July 27, 1910. They lived there until 20 years ago when they moved to Centerville. He was a coal miner and was a fireman for the Kemble Floral Co. here when he retired last February because of ill health. He was a long-time member of the Gospel hall group here.
He is survived by his wife; two daughters, Misses Mildred and Edna Lewis at home; two sons, John D. Lewis of Centerville and Paul Lewis at home; two grandchildren, four sisters, Mrs. Glen Johnson and Mrs. Jemima Van Dorn of Rathbun, Mrs. Matthew Horn of Waukee, Iowa, and Mrs. Ann Herrin of Galesburg, Ill, Two brothers, David Lewis of Rockford, 111., and Thomas Lewis of Waukee.

He was preceded in death by a sister and a brother.

Services will be held at the Miller funeral home at 2 p. m. Wednesday, in charge of Evangelist John Horn of Atchison, Kan., and Evangelist David Horn of Denver, friends of the Lewis family. Evangelist David Lawrence of Centerville, pastor of Gospel hall and Mrs. Hugh Guernsey will take part in the service the pallbearers will be nephews Burial will be in the Oakland cemetery.
