Jay Thomas Allen, Our Father, age 64, passed away May 6, 2002; born in Fort Dodge, Iowa; is survived by 2 brothers, 2 sisters, his mother (Frances), 5 children and 7 grandchildren. Services to be held at Unity of Phoenix, 1500 East Greenway Parkway at 4pm on Thursday the 9th of 2002. Go with Marge, rest in Peace.
