23 December 2003Teresa M. Forman, 32, of Colorado Springs, Colo., formerly of Sioux City, died Friday, Nov. 28, 2003, in Colorado Springs.
Services were held at St. Michael's Catholic Church in South Sioux City. The body was cremated in Colorado. Local arrangements were under the direction of Christy-Smith Morningside Chapel.
Teresa was born Jan. 15, 1971, in Wagner, S.D., the daughter of Randall Baker and Beverly Forman Trizila. She moved to California, where she lived before moving to Sioux City in 1987. Following her education, she moved to Colorado in 1990.
She loved the animals of Colorado and all of outdoors and enjoyed traveling. She was a very artistic person and did drawings.
She is survived by her mother and stepfather, Beverly and Denny Trizila of Sioux City; two brothers, Robert J. Dalton and J.G. Ohler and his wife, Cindy of Vermillion, S.D.; a sister, Tiffany Casares of Sioux City; a brother-in-law, Vicente Pajarito of Guadalajara, Jalisco, Mexico; two stepsisters, Lisa Poff and her special friend, Brad Allen of Jefferson, Iowa, and Dawn and her husband, Brian Hindman of Pierre, S.D.; a great-aunt, Thelma Welch of Sioux Falls, S.D.; an aunt and uncle, Gwen and Jerry Sparck of Crescent, Iowa; a stepgrandmother, Mildred Forman of Orlando, Fla.; nieces and nephews, Karisa, Sarah and Rudy Ohler, Denny and Chantel Trizila, Jennifer, Stephanie, Nolan and Steven Hindman and John, Justin and Jerica Baker; and her faithful friends, Ashley her dog and Crackers Ann her cat.
She was preceded in death by her father, Randall Baker; and maternal grandparents, Clarence and Dorothy Forman.
