Phillip Hayek

Phillip Allen Hayek, 8, of rural Fort Atkinson, died Monday, Aug. 28, 2000, at the Gundersen Lutheran Medical Center in La Crosse, Wis.

A funeral mass was held Friday, Sept. 1, at 11 a.m., at St. Wenceslaus Catholic Church in Spillville, with the Rev. Donald Hawes officiating.

Doris Thompson was the organist. Dan Silhacek was the soloist.

Edward Hayek Jr. was the lector. The offertory was received by Jessica, Jamie and Jodi Balk. Servers were Wayne and Maria Kovarik.

Casket bearers were Leo Balk, Gerald Balk, Paul Farley and Dennis Farley. Honorary casket bearers were Erica Dahl, Shane Dahl, Steven Dahl, Brandon Dahl, Clifton Baker, Colton Baker, Clayton Baker, Christian Baker and Nicole Hayek.

Burial was at the St. Wenceslaus Cemetery. The Balik Funeral Home handled arrangements.

Phillip was born Dec. 10, 1991, at the Gundersen Lutheran Medical Center Union. He received his First Communion last year at St. Wenceslaus Church. He liked flowers, beanie babies, rocks, fishing and cars and was active in 4-H.

This year Phillip was in the third grade at South Winneshiek Elementary School in Ossian.

Survivors include his father Daniel and step-mother Marleen Hayek of Fort Atkinson; his mother Nancy and step-father Rick Baker of Maynard; sister Nicole Hayek of Maynard and step-sister Erica Dahl of Fort Atkinson; half-brother Colton Baker; step-brothers Chris, Kit, Clifton and Clayton Baker of Maynard; and step-brothers Steven, Shane and Brandon Dahl of Fort Atkinson.

Also surviving are his grandparents, Edward and Marcella Hayek of Spillville, and Allen and Naomi Balk of Waucoma; step-grandparents Ruth Farley of Lansing, and Clifford and Mary Lou Baker of Maynard; great-grandparents Evelyn Balk of Lawler and Evelyn Miller of Hawkeye; step-great-grandparents Ralph and Angie Roney of Decorah, and Angle Farley of DeWitt, along with many aunts, uncles and cousins.

A funeral mass will be held Friday, Sept. 1, at 11 a.m., at the St. Wenceslaus Catholic Church in Spillville with the. Rev, Donald Hawes officiating.

Burial will be at the church cemetery. The Balik Funeral Home is handling arrangements.

Visitation will be held at Balik's at 5 p.m. and a Prayer Service at 7 and at the church Friday after 10 a.m.
