Born July 14, 1895, Elberon, Iowa.

Son of Lewis and Anna Kubik.

Enlisted July 24, 1918 at Cedar Rapids; entered Camp Pike. First entered 38th Co., 10th Rec. Bn., 162nd Depot Brigade.

Mustered out Jan 15, 1919 from Co. D, 4th Reg. Inf.
