You will want to go to the Iowa Department of Public Health web site.

From their you can download a application to send in to them along with a check for $15. It list the requirments their.

1901 is pretty old though. I have had some luck and alas some where I wasnt lucky.

The $15 dollars is used up whether they find it or not.

If they dont find it only option is to county court house and get a copy from the death register of the listing.
