Confirm date and place of birth and death

I would like to confirm the date and place of birth, date of death, and place of burial for John Olsasky (might also be Olsavsky). The information I have says he was born on January 3, 1890. I would like to confirm this and also get the place of birth (could be a mining camp -- possibly Carbanado). The date of death I have is January 14, 1890. However, the family history I have says that he died at birth, so the date of death could have been January 4 rather than January 14. I believe that he is buried at Old White Cemetery and would like to know if this is what the death record says. Thank you to anyone who can help me!
