Phyllis McCulloh, 81, died Tuesday, January 21, 2014, surrounded by her family at Methodist West Hospital in West Des Moines. Memorial services will be held 11 a.m. Saturday, January 25, at Iles-Westover Chapel, where the family will receive friends one hour prior to the service.

Phyllis was born March 19, 1932, in Madison County, Iowa to Forrest and Roxie (Beach) Dale, the youngest of 11 children. She married Dale Crow in 1951 and bore three children before divorcing. Phyllis later married Ellsworth McCulloh in 1968 and had one son. She worked for Northwestern Bell in Des Moines before moving to Illinois where she worked for GE until she retired.

Phyllis is survived by her children Dan (Stacey) Crow of Cumming, Brenda (John) Cavallo of Boca Raton, FL and Timothy Ellsworth McCulloh of Des Moines; her grandchildren, Toby Hicks, Sean Crow, Ryan Crow, Shannon Nehrkorn, and Olivia Cavallo; six great-grandchildren, and her brothers, Donald Dale of Osceola, and Wayne Dale of Indianola. She was preceded in death by her parents, husbands, daughter, Marcia Ortega, and five brothers and three sisters.
