Lee Co., Iowa has two county seats. One in Fort Madison, IA and the other in Keokuk, IA.

In Fort Madison the Recorder's office is located at 10 St & Ave H, 319-372-4662 and in Keokuk the Recorder's office is located at 25 N. 7th St 319-524-1126.

Also, since your relative is buried in Farmington, IA it could be that the death occured in Clark Co., MO. County Seat there is Kahoka, MO. Farmington is very close the the border of MO.
