Jerome "Jerry" Forrest Kurtz, 66, and Carolyn L. Craig Kurtz, 67, of Payson, Ariz., died Monday, May 3, 2004, in a small-plane crash in Cary, N.C.

A funeral Mass will be held at 9:30 a.m. May 29 at St. Paul's Catholic Church in Phoenix, Ariz.

In lieu of flowers, memorials can be sent to Catholic Charities USA, PO Box 25168, Alexandria, Va. 22313.

Mr. Kurtz was born March 3, 1938, in Pleasant Plain, the son of Jerome and Myrtle Kurtz.

He was reared in Pleasant Plain. He served in the United States Army from 1958 to 1961.

Mrs. Kurtz was born March 13, 1937, in Illiopolis, Ill., and was reared in central Illinois.

The couple was married Oct. 6, 1962 in Chicago.

After moving to Greybull, Wyo., in 1969, they owned an auto dealership for five years, and opened Woodpecker Products Inc., a cabinet and woodworking business, in 1974. In 1979, the business was moved to Montrose, Colo., and was sold in the early 1990s. The couple relocated to Payson, where they developed the Matazal Mountain Air Park.

Mr. Kurtz earned his private pilot's license in the early 1990s. The couple traveled extensively throughout the United States, visiting family and friends and sightseeing.

Mr. Kurtz was a member of the Rotary Club, the Chamber of Commerce and the Knights of Columbus.

Survivors include two sons, Craig Kurtz and wife Karen of Phoenix and Christopher Kurtz and wife Erin of Windsor, Colo.; three grandchildren; Mr. Kurtz's two sisters, Jan Poggenpohl of West Warwick, R.I., and Susan Baker of Brighton; and Mrs. Kurtzs sister, Imogene Berger of Chicago.
