Shadam Nolan Hazelwood, 11-month-old son of Joseph and Stevie (Holzhauser) Hazelwood of 1319 Glenway in Newton, died Tuesday, May 3, 2005, at the Skiff Medical Center in Newton.

Funeral services will be at 1 p.m. Friday at Reece Funeral Home in Ottumwa with the Rev. Charles Grove officiating. Cremation will follow the services.

Visitation will begin at 2 p.m. Thursday with the family present from 5 to 7 p.m. today. Memorials may be made to the family.

He was born May 18, 2004, in Des Moines.

Surviving are his parents of Newton; grandparents, Denise and Kevin Holzhauser of Agency and Connie and Will Hazelwood of Ottumwa; great-grandparents, Mary Ellen Lunkley, Reva Holzhauser, Bonnie and Bill Hazelwood and Virginia and Chuck Barwick, all of Ottumwa; and great-great-grandmother, Maude Renfrew of Ottumwa.

He was preceded in death by great-grandfathers Jack Lunkley and Harold Holzhauser
