ETHEL M. FORMAN

EPWORTH, Iowa - Ethel M. Forman, 90, of 23872 Mill ville Road, Epworth, died at 1:30 p.m. Thursday, Jan. 13, 2000, at Mercy Medical Center-Dubuque.

Services will be at 10:30 a.m. Monday at St. Patrick's Catholic Church, Epworth. Burial will be in Mount Olivet Cemetery, Key West, where military rites will be accorded by John White American Legion Post 656 of Epworth. Friends may call from 2 to 9 p.m. Sunday at Reiff Funeral Home, Epworth, where there will be a wake service at 7:30 p.m.

Mrs. Forman was co-administrator of the former Dubuque County Nursing Home from 1959 to 1974. She then worked as a dietitian at the Dubuque County Home and later at St. Dominic's Villa, until retiring in 1985.

She was born on May 5, 1909, in Dubuque, daughter of Charles A. and Loretta (Brummer) Kress. She married Clarence Forman on June 17, 1946, in Nativity Catholic Church, Dubuque; he died in May 1978.

She was a graduate of the former Visitation Academy, Dubuque, and of Clarke College, Dubuque, where she received her degree in dietetics nutrition. She was a member of St. Patrick's Parish, Epworth, and of John White American Legion Post 656 of Epworth.

She served in the Army from March 15, 1943, until July 2, 1946, working as a dietitian in field hospitals overseas during World War II. She received the Meritorious Unit Badge, Battle Star, American Theatre Ribbon, Victory Medal, EAME Theatre Ribbon with a bronze and two overseas service bars.

Surviving are two sons, James Forman, of Epworth, and Charles (Jolyn) Forman, of Decorah; three grandchildren and a great-grandchild; a sister, Sr. Justina Kress OP, of St. Dominic's Villa, Dubuque; and a niece, Barb Stevenson, of Colley Ville, Texas.

She also was preceded in death by her parents; a son, in infancy; a daughter, Ruth Ann Forman; and two brothers, Cliff Kress and Monsignor Justin Kress.

An Ethel M. Forman Memorial Fund has been established.
