This was posted many years ago but I will respond anyway. I have tracked Frank J Guth to Salt Lake City where he purchased property. His first wife, Mary either died or they divorced because she did not go to Utah. I believe she was buried and/or stayed in Pennsylvania (I have an Army record of his pension payments with her address). I have no information regarding her death except in the Army record saying she as the beneficiary died 19 February 1912 which may or may not be accurate. I don't have any other documentation on that.
He had a 2nd wife Julia Besonette with him and she is buried in a Catholic cemetery in Salt Lake. I found documents attached to his property in Salt Lake County records listing his death date as 5 December 1913. It is my guess that he was visiting Mary in Iowa and possibly had a stroke or something but was disoriented and picked up by the "Sanity Commision" in Rockville, IA (found in the Salt Lake Tribune @ chroniclingamerica.loc.gov on 6 May 1913-the link is below). The property documents state he died in the Calhoun County insane asylum (they charged his estate $72.35 for care). I am happy to share any of my documentation.
I would love to know where he is buried. I have called different places with no luck. I hope this is helpful.
