Hazel L. Cook-Coffin, 95, a resident of Friendship Manor, Rock Island, Ill., died Wednesday, July 20, 2005, at the Silver Cross Nursing Center, Rock Island.

Services will be 11 a.m. Monday, July 24, 2005, at Good Shepherd Presbyterian Church, Rock Island. Visitation will be one hour before services at church on Monday, beginning at 10 a.m. Graveside services will be 1 p.m. Tuesday, July 25, 2005, at Fairview Cemetery, Cedar Falls. Memorials may be made to Friendship Manor, Good Shepherd Presbyterian Church or to Pathway Hospice. Wheelan Funeral Home, Rock Island, is assisting the family with arrangements.

Hazel was born July 27, 1909, in Cedar Falls, the daughter of Clinton and Nina Miller Weisbard. Hazel married John Cook on Oct. 15, 1931, in Minneapolis, Minn. He preceded his wife in 1975. She later married John Coffin on April 2, 1976, in Washington, Iowa; he also preceded his wife in 1992. Hazel worked as a homemaker. Previously she attended the University of Northern Iowa, graduating in 1961 with a bachelor's degree in elementary education. She taught for 8 1/2 years in the Cedar Falls area. Hazel's memberships include Good Shepherd Presbyterian Church, Rock Island, or First Presbyterian Church, Cedar Falls.

Hazel was an active and devoted member to the churches she served for many years in different capacities. She also was a member of the Cedar Falls Woman's Club; King's Daughter's - Victory Circle, Rock Island; and Order of the Eastern Star, Cedar Falls. Hazel enjoyed flower arranging, golf, gardening and playing bridge and especially her grandchildren and great-grandchildren.

Survivors include her children, Charlane Hansen of Blue Grass, Sally and Dr. Harold Klepfer of San Jose, Calif., and John and Bonnie Cook of Marietta, Ga.; 10 grandchildren and 19 great-grandchildren; and many nieces and nephews.

Mrs. Cook-Coffin was preceded in death by her parents, brothers and sisters.
