WATERLOO - Lori Kay Formanek, 50, of Waterloo, formerly of Chelsea and Tama, died Saturday, June 8, 2013, at University of Iowa Hospitals and Clinics, Iowa City, after a short illness.

Survived by a daughter, Sterling Formanek of Chelsea; and five brothers, Calvin (Deb) of Mankato, Minn., Maynard (Pam) of Garwin, Marlin (Laurie) of Urbandale, Richard (Joan) of Piney Flatts, Tenn., and Pat (Diana) of Belle Plaine.

Preceded in death by her parents, Harold and Bernice Thelen Wedmore.

Services: 10:30 a.m. Thursday at St. Patrick’s Catholic Church, Tama, with burial in Maple Hill Cemetery, Gladbrook. Visitation will be from 5 to 8 p.m. Wednesday at the church. Hrabak Funeral Home, Belle Plaine, is in charge of arrangements.
