Maybe the stone in Appanoose county was done when Archie died as a family stone and her death didn't actually occur there to be added.

In the 1910 census for Johns Twp, Appanoose county I found Elizabeth living with James M. and Ellen E. Johnston, she listed as mother-in-law. I couldn't find her in the 1920 census, but James M and Ellen E are listed in the 1920 and 1930 censuses in Ashby, Grant County, Nebraska, the same location as the Nebraska gravestone. Maybe she went to live with them in her last days and actually died in Nebraska.

This page gives information on how to obtain a death certificate in Nebraska.
