Diana, this is probably more than you need but I made notes as I looked for the answers to your questions.
It seems 'very likely' that he's buried in the Waterville Lutheran cemetery, but there may not be a gravestone. The family used both Hjornvig / Hjornevig and Larson as surnames, the former likely being the Norwegian farm name or town in Norway. Otto's death certificate (below) used Hjornvig.
-1920 U.S. census, Paint Creek twp:
Hjornevig, H.J.; HOH, age 57, born IA of Norwegian born parents
Hjornevig, Mattie; wife, age 47; born IA of Norwegian born parents
Children, all w/surname Hjornevig and born in IA:
Alfred (23), Helen (20), Otto (18), Ingvald (16), Malvin (15), Emma (13), Bertha (10), Clarence (5mo)-H.J. Hjornevig named as parents in 1920 Federal census, but in the 1925 IA State census as Louis L. & Mathea Hjornevig. Other records father as Louis Larson. Again, likely due to Norwegian naming practices. In the 1925 both Otto and Helen are absent (as well as older brother Alfred). Hellen Larson died in October 1923 and her death cert. gives burial in Waterville Lutheran cemetery. The 1925 census gives parents of Lewis as Lars Hjornevig and Julia Seim (best guess) and of Mathea as Andrew Anderson & Bertha Granisum (best guess). Both Louis L. & Mathea Hjornevig are shown in the 'Woodmansee' cemetery book as buried in Waterville Lutheran.
-In an article printed in the Fayette County Leader, 7/12/1923, pg 1, was the following paragraph:
Otto Larson, a son of Mr. and Mrs. Louis Larson of Waterville, suffered the greatest injury, his spine being injured and he has since been paralyzed from the waist down. His recovery is regarded as doubtful.

Several from here attended the funeral of Otto Larson at Waterville on Friday last.And on pg 4, same paper & date:
Otto Larson, the Waterville young man who had his spine injured when the runway leading to a barn near McGregor in which a dance was being held collapsed on the evening of the Fourth of July, died in the McGregor hospital and the remains were brought to Waterville Thursday.-In an article printed in the Dubuque Times-Journal, 7/20/1923, pg 14, was the following paragraph:
Waukon - Fall Proves Fatal. The funeral of Otto Larson, 21, injured in a fall at a barn dance on July 4 and who died in a hospital at McGregor, was held today at Waterville.-Death Certificate (familysearch.org)
