The United States Army, as a Testimonial of Honest and
Faithful Service, is hereby Honorably Discharged from the
military service of the United States by reason of: 9th
Ind. Hq. 34th Division, March 13, 1918
Said Thomas I. O'Brien was born in Waukon Junction in the
State of Iowa. When enlisted he was 26 years of age, and
by occupation a Farmer. He had Blue eyes, Dk. Brown hair,
Fair complexion and was 5 feet 9 1/2 inches in height.
Given under my hand at Camp Cody, New Mexico this 15th
day of March, one thousand nine hundred and eighteen.
