Know ye, That Harvey S. Curtis a Private of Captain J.W. Bice Company, (A), 3rd Regiment of Cavalry Illinois VOLUNTEERS, who was enrolled on the 14th day of August one thousand eight hundred and Sixty one to serve Three years, or during the war, is hereby Discharged from the service of the United States, this fifth day of September 1864, at Springfield Illinois by reason of expiration of term of service

[No objection to his being re-enlisted is known to exist.)

Said Harvey S. Curtis was born in Napoli Cott in the State of New York , is 21 years of age, 5 feet 7 inches high, light complexion, hazel eyes, brown hair and by occupation when enrolled a teacher.

Given at Springfield Ill., this fifth day of September 1864.
