Instructions for Submitting Your Family Groupsheet

The chart below is made for a family of 15 children. To simplify filing these and prevent duplication, we will only be filing the chart under the *father's* surname. We hope that you will also submit a separate chart on the mother's family. If there is more than one marriage, then please do another group sheet and number it as #2 by the husband's name.
Please do not send attachments without prior notice.
I will not accept submissions that do not include
the submitter's name and email address.

  • Do NOT include living persons on these charts (they will not be added). You may put LIVING in the space for a person's name, but no other information.
  • Put all SURNAMES in capital letters.
  • List all women under their maiden names; if unknown, then please list as such.
  • Add any additional information in the area left for notes below the children.
  • If you can't use this electronic form, then copy and paste the alternate email form into an E-MAIL and fill in the blanks.
  • If your software can generate a family group sheet report in .txt format, you may send that. I can only accept text (.txt) files. Do not send me any other format.
  • You are strongly encouraged to quote your sources.
  • Use your TAB key to move through the form.
  • Do not hit ENTER until you are ready to submit your data.
