#3224213   Private Company "C" Development Battalion No.1

          

THE UNITED STATES ARMY, as a Testimonial of Honest and Faithful Service, is herby HONORABLY DISCHARGED from the military service of the UNITED STATES by reason of    Physical deficiency per Surgeon's Certificate of Disability and 4th Ind. 163rd Depot Brigade October 22, 1918 giving as cause for discharge Neuro-Circulatory Asthenia which incapacitates by reason of tachycardia, dizziness and palpitation on exertion rendering him liable to collapse. Discharge by Order of the Secretary of War Par. 7, General Order 45 War Dept. 1918.

in the State of    Iowa When enlisted he was 22 7/12  years of age

and by occupation a   Farmer

He had  blue   eyes,  dark  hair, ruddy  complexion, and was   five   feet   five and one half  inches in height.

      Given under my hand at    Camp Dodge, Iowa.
