B12. 152nd Depot Brigade
was discharged from the military service of
the United States by reason of physical disability at Camp Upton,
N.Y. on the twenty-eighth day of June 1918, that at the time of
discharge there was erroneously issued to him a discharge
certificate on Form525, A.G.O., indicating that he had been
Honorably Discharged the service. Having surrendered said
certificate so issued to him, this certificate is issued in lieu
thereof, and by virtue of this certificate he is entitled to be
regarded as having been Honorably Discharged the military service
as of the date and place aforementioned.
