This is to certify That No. Carl Henry Gaulke a Fireman, Second Class, U. S. Navy has this day been discharged from the U. S. Naval Training Station, Newport, R. I. and from the U. S. Naval Service, by reason of his own request, prior to expiration of enlistment by SOSN (without refund) enter expiration of enlistment physical disability special order of the Navy Department (under age) & by order of Bureau of Navigation, in accordance with BN C/L 3-19 of 6 January 1919. Waived all claim to transportation to Naval Station nearest his home (date); or other reason, giving same in detail.
