appointment will be paid by the Medical Department and
noted on this letter of appointment. |

Vouchers for pay will be made out on Form
335. When base pay $50 a month only is allowed the entries
on the face of the form will suffice. When additional pay
is allowed the whole amount due should be indicated on the
face of the form, and the items thereof should be indicated
in the second fold of the form thus, e. g.: "Base pay from
----to----," Additional pay in lieu of lodgings and meals,
from ----- to-----$------; Additional pay for services
beyond seas at----, from---- to----$----; Additional pay as
Head Aide from ----to----$----"; Item additional pay in
lieu of lodgings and meals must be supported by the
responsible Officer's certificate that it was not
practicable to lodge her at the hospital. |
