THE UNITED STATES ARMY, as a Testimonial of Honest and Faithful Service, is herby HONORABLY DISCHARGED from the military service of the UNITED STATES by reason of     Physical deficiency per Surgeon's Certificate of disability and 4th Ind. 163rd Dept Brigade November 4, 1918, giving as cause for discharge neuro-circulatory asthenia which incapacitates by reason of tachycardia, dizziness and palpitation on exertion, rendering him liable to collapse. Discharged by order of the Secretary of the War, Paragraph 7, General Order, War Dept. 1918.

in the State of    Iowa When enlisted he was    22 2/12     years of age

and by occupation a   Barber and Farmer

He had  blue  eyes,     light  hair,     fair  complexion, and was  five   feet  eight & 1/2  inches in height.

      Given under my hand at    Camp Dodge, Iowa

This    6th   day of     November,    one thousand nine hundred and     eighteen.
