Phyllis Shook, 65, of Johnston, died on Friday, January 16, 2004. Funeral services were held on Monday, January 19, at the Johnston Evangelical Free Church. A chapel and graveside service was held on Tuesday, January 20, at the St. Olaf Lutheran Church in rural Belmond. Phyllis was born on December 30, 1938 in Belmond, the daughter of Archie and Hilda Lee. She and her husband, Bill founded AP/M Permaform in Johnston. She was preceeded in death by her parents; and sisters, Donna Goeman and Lavonne and Lois Lee. Survivors include her husband; daughter, Lori (Kenny) Horns of Minnesota; sons, Jeff (Mildred) Severseike of Missouri and Mark (Paula) Severseike of Arizona; sister, Leone (Leo) Weinheimer of Greenfield; and several nieces and nephews in the Belmond area.
