Charles J. “CJ” Forman, age 64, of Decorah, IA, died Saturday evening, June 28, 2014 at home in Decorah, in hospice care.

Visitation will be from 4:00-8:00 p.m. Tuesday, July 1 at Fjelstul Funeral Home in Decorah. Additional visitation will be from 11 a.m. to 1 p.m. Wednesday, July 2 at Behr Funeral Home in Dubuque, IA. Entombment will follow at Mt. Calvary Cemetery in Dubuque.

Charles Justin “CJ” Forman was born on June 29, 1949 to Clarence and Ethel (Kress) Forman in Dubuque, Iowa. He graduated from Wahlert High School in Dubuque with the class of 1967 and from Loras College in Dubuque with a B.A. degree in Accounting with the class of 1971.

CJ married Jolyn Perrott on April 28, 1973 in Dubuque, Iowa. Their union was blessed with three children: Catherine, Andrew and Christopher.

He worked for People’s Natural Gas, which became Black Hills Energy, in Dubuque and they transferred him to the Decorah office in 1986. He worked in a variety of positions, most recently as a Service Specialist. CJ retired in February 2010 after 38 years with the company. He was a volunteer with the Decorah Fire Department for 18 years, volunteer camera operator for the Decorah Eagles, and a Red Cross volunteer in Decorah.

He was preceded in death by his parents, Clarence and Ethel, and an infant sister, Ruth Forman.

CJ is survived by his wife Jolyn; one daughter, Catherine Forman (Craig Erickson) of Decorah; two sons, Andrew (Tammy) Forman of Hazel Green, WI and Christopher Forman of Waterloo, IA; one granddaughter, Lexee Forman; two grandsons, Griffin Forman and Dude Dagel; one brother, James Forman of Epworth, IA; one cousin, Barbara Stevenson of Colleyville, TX; three sisters-in-law, Nina Perrott (Dennis Horch), Susan (Paul) Kurt and Carla (Ron) Klosterman, all of Dubuque; one brother-in-law, William (Denise) Perrott of Waverly, IA; and numerous nieces and nephews.
