Obituary From “Ottumwa Courier,” January 23, 1975:

Mrs. Hazel B. Thomas, 61, of 319 N. Johnson, was dead on arrival Wednesday at 10 a.m. at the St. Joseph Hospital following a heart attack at her home.

She had been a teacher in both rural and public schools for many years, most of the time at Douglas School, and since 1960, she was a special education teacher at the Orchard School.

She graduated from Ottumwa High School in 1931 and from Parsons College in 1960.

She was born May 15, 1913, in Wapello County, the daughter of Fred and Tillie Forsyth Ross. She had been a resident of Ottumwa most of her life. She was a member of the Davis Street Christian Church.

She was married to Leroy L. (Pete) Thomas, Oct. 5, 1945, in Ottumwa. He survives. Also surviving are two sons, Fred of Ottumwa, and Gene, a student at the University of Iowa; one daughter, Mary Jane Dail of Swayzee, Ind.; one granddaughter; and one sister, Iva M. Smith of Rock Island, Ill.

She was preceded in death by an infant brother.

The funeral will be Saturday at 1:30 p.m. at the Johnson Funeral Chapel, with the Rev. Cecil Simonton of the Davis Street Christian Church officiating.

Burial will be in the Shaul Cemetery.

The service for Mrs. Hazel B. Thomas, 61, of 319 N. Johnson, who died Wednesday, will be held Saturday at 1:30 p.m. at Johnson Funeral Home. The Rev. Cecil Simonton of the Davis Street Christian Church will officiate. Burial will be in Shaul Cemetery.

Pallbearers will be Ernest Leonard, Russell Jackson, Robert W. Hart, Jack Hampton, Paul Matthew and Sam Kinsinger.
