Parents: Mr. and Mrs. Frank Kubik

Left for Overseas: May 1918; assigned 33rd or Prairie Div.

Medals-Ribbons: Twice cited for gallantry and splendid performance of duty, one citation signed by Gen. Pershing

Died: Killed in action in France on Aug. 9, 1918 in Battle of Chipilly Ridge; buried in American Military Cemetery at Vaux-sur-Somme.
