Son of Louis and Fannie Vlasak.

Entered service Sep 5, 1918, Camp Dodge; transferred to Philadelphia Hosp. No. 22; first entered Co. 47, 12th Bn., 163rd Depot Brigade.

Mustered out June 14, 1919 from Medical Dept.
