Born Sept. 25, 1891, in Carroll Township, Tama County, Iowa.

Son of Joe and Anna Z. Kunch Machacek.

Entered service July 25, 1918.

Entered Camp Gordon; transferred to Camp Merritt; first entered Co. C, 4th Inf. Repl. Reg.

In France Sept. 12, 1918; member of 314th M.G., 79th Div. In battles of Meuse-Argonne Offensive.

Killed in action Nov. 5, 1918, near town of Verviers.
