Son of Samuel E. and Sarah E. Levis.

Enlisted July 24, 1918 at Toledo; entered Camp Gordon; first entered Co. C, 4th Inf., Repl. Reg.

Mustered out April 21, 1919 from Base Hospital.

In France Sep 1918. Wounded behind the lines in the right hand finger and part of hand is missing.
