Son of Joseph and Anna Klemak.

Enlisted May 30, 1918, at Marshalltown, IA.

Entered Ft. Logan; transferred to Presidio. Ft. Baker and Camp Eustis, Camp Stuart; first entered 19th Co.

Mustered out Feb 25, 1919 from 46th Artillery, Battery D. C.A.C.

In France Oct 26.
