Born April 21, 1896 at Clutier, Iowa.

Son of John and Anna Cizek.

Entered service May 10, 1918.

Entered Jefferson Barracks, Camp MacArthur, and Camp Merritt; entered Recruit Camp, 7th Div., Camp MacArthur, Co. 23.

In France Aug 10, 1918. In Co. I, 56th Inf. and 4th Div.; Co. D, 39th Inf. while in France. Was sereiously wounded in neck and shoulder. In battles of St. Mihiel and Verdun.
