Son of C. H. and Qua-che-wa Chuck.

Enlisted May 27, 1918 at Toledo, IA.

Entered Camp Gordon; first entered Co. B, 10th Inf. Training Bn., 4th Repl. Reg.

Mustered out Jan 9, 1919 from Co. 9, 3rd Bn., 163rd D.B.

Sergeant.

Thomas Chuck was an Indian of Tama Rerservation.
