Kirk C. Koithan, D.O., 49, of Hinton died Wednesday, Dec. 3, 2008, at a Sioux City hospital following a lengthy illness.
Services will be 11 a.m. Monday, December 8, 2008 at Calvary Lutheran Church in Sioux City, with the Rev. James Travis officiating. Burial will be in Mt. Hope Cemetery at Mapleton, Iowa. Visitation will be 3 to 8 p.m. today, with the family present 6 to 8 p.m. and a 7 p.m. prayer service, at .
Kirk was born Nov. 19, 1959, in Battle Creek, Iowa, to George and Evelyn (Justi) Koithan. He grew up in Mapleton, Iowa, and graduated from Maple Valley High School with honors. He graduated from Iowa State University and received his D.O. degree from the University of Osteopathic Medicine and Health Sciences in Des Moines. He served his internship and residency at Mt. Clemens General Hospital in Mt. Clemens, Mich.
Kirk married Cheryl A. Hildebrand on June 18, 1983, in Washington, Iowa. He was a family practice physician and practiced at the Fort Dodge Medical Center from 1989 to 1996 and then practiced in South Sioux City from 1996 to 1998, when he retired due to ill health.
He was a member of the American Osteopathic Association, American College of Osteopathic Family Practitioners, American Academy of Family Practice, Iowa Osteopathic Medical Association and the Iowa Medical Society. He was board certified in the American Osteopathic Board of Family Practitioners. He was a member of Calvary Lutheran Church.
Survivors include his wife, Cheryl of Hinton; two sons, Douglas J. Koithan and Stephen J. Koithan, both of Hinton; a daughter, Sarah E. Koithan of Hinton; his father, George Koithan and his wife Irene of Carroll, Iowa; two brothers, Daniel Koithan and his wife, Carol of Mapleton, and Dr. Thomas Koithan and his wife, Rose of West Des Moines.
He was preceded in death by his mother, Evelyn.
Pallbearers will be Steve Nieland, Mike Flanigan, David Koithan, Jeffrey Koithan, Jim McKune and Mark Rhoads. Honorary pallbearers will be Jim Koithan, Benjamin Koithan, Eric Koithan, Jeff Parker, Dan Johnson, Joe Krug, Steve Dullard, John Goodlove, Tim Beckwith and Dennis Justi.
Memorials may be directed to Hospice of Siouxland or Calvary Lutheran Church.
