Phyllis Theobald, 84 of Lone Tree Care Center formerly of 530 S. Dodge, died Wednesday, November 12, 2003 at the University of Iowa Hospitals & Clinics.

Phyllis was born August 11, 1919 in Iowa City, Iowa the daughter of John L. and Mary Ellen (Burns) Theobald. She attended St. Patrick’s School. She was a member of St. Patricks Catholic Church. Phyllis was a resident of Lone Tree Care Center for the last six years.

Phyllis was survived by five nieces and nephews and numerous great nieces and nephews.

She was preceded in death by her parents, two brothers, John and William, three sisters, Mary Theobald, Marguerite Thyne, Lucille McComas; one niece Gwen Cooper and one great niece Elizabeth Parent; and one great nephew Steve Cooper.
