James Formanek, 49, died May 31 while fishing at Crystal Lake,

Funeral services held at St. Wenceslaus Catholic church, Duncan.

Survived by his wife, Agnes, 4 children, Richard and Mrs. Joyce Moen of Minneapolis, Evelyn of Mason City, and Douglas at home, 5 brothers, Louis of Garner, Joe of Hayfield, Frank of Garner, George of Britt and Ed of Garner, 2 sisters, Mrs. John Nedved of Garner and Mrs. Gerald Tupy of Lawler.
