Chad M. Cousins, 29, of DeWitt, Iowa, died Sunday morning, May 15, 2016 in Davenport.

Chad Michael Cousins was born November 19, 1986, in Davenport to Terry Cousins and Martha (Lake) Cousins. He attended school in DeWitt. He loved his nieces and nephews dearly.

Surviving are his parents, Terry Cousins of Clinton and Martha Cousins of DeWitt; siblings, Eric (Amber Bauer) Cousins, DeWitt, Tera Cousins of Davenport; step-siblings, Sebastian Cousins and Dawn Twigg; his grandma, Thelma Butler of DeWitt; aunts and uncles, Mike (Teri) Lake, Mark (Jean) Lake, Brian (Debbie) Lake and Steven (Chris) Lake, Judy Jansen and Cheryl (Martin) Andersson.

Preceding Chad in death were his grandparents, Clement and Evelyn Cousins, Dean Lake and Charles Butler and a step-brother, Tobias.

Visitation will be at Schultz Funeral Home, DeWitt, from 4:00 until 7:00 p.m. Friday, May 20th. A service will be held at 10:00 a.m. Saturday, May 21, 2016, in the funeral home. Burial will be at Elmwood Cemetery, DeWitt.
