CEDAR FALLS- Hazel Marie Kozak, 83, of Cedar Falls, died Sunday, April 12, 2009, at Sartori Memorial Hospital.

She was born November 7, 1925 in Oelwein, the daughter of Merle and Ruth (Garrison) Fox. She married Sylvan Livingston on January 11, 1946 in New Orleans, and they later divorced. She then married Anthony "Tony" Kozak on December 31, 1966 in Galena, IL.

Mrs. Kozak owned and operated the Blue Room, then worked in housekeeping at the Black Hawk Hotel, and was later employed as the Head Cook for the Senior Activity Center in Waterloo.

Survivors include her husband, Tony; four daughters: Sylvia Alberts of Las Vegas, NV, June Zmolek of Clarksville, Mary Lou (Dennis) Kruse of Cedar Falls, and Patricia (Mike) Byrnes of Waterloo; eight grandchildren, Jennifer, Jesse, Katie, Rosie, Faith, Leah, Marshall, and Lexi; and seven great-grandchildren.

Preceded in death by a grandson, Dustin Finger; and two brothers, Willis and Kenneth Fox.

Services will be held 10:30 a.m. Thursday at Richardson Funeral Home, with burial in Cedar Valley Memorial Gardens, both in Cedar Falls. Friends may call from 4 to 7 p.m. Wednesday at the funeral home.
